This patient has unsustained left ankle clonus in response to forced ankle dorsiflexion by the examiner. A corticospinal tract lesion may result in extreme hyperreflexia with ankle clonus due to disinhibition of the spinal reflex arc. Like the extensor plantar response, clonus is a sign of upper motor neuron dysfunction in the brain, brainstem or spinal cord.
