Cardiac transplantation alone is not an appropriate option in the treatment of idiopathic pulmonary hypertension where the problem is with the pulmonary vasculature. Placement of a new heart would result in acute right heart failure, as it would be exposed to significantly elevated pulmonary artery pressures. Cardiac transplantation combined with lung transplantation was the previous treatment of choice in idiopathic pulmonary hypertension in patients who failed medical therapy.
