The diagnosis of musculoskeletal pain is supported by the patient's typical history of chest and shoulder pain that began after strenuous exercise of the left arm and became worse with arm movement. His physical examination revealed tenderness of the left chest and shoulder area, and was otherwise within normal limits.

While a normal physical examination does not rule out heart disease, the recognition of normal is critical to the recognition of pathology at the bedside.
