Open mitral commissurotomy is the treatment of choice for those patients who are not candidates for balloon valvuloplasty, but do not require valve replacement. For example, a typical patient might have failed balloon valvuloplasty, or might require concomitant surgery for aortic valve replacement. Surgery may also be considered in a patient with left atrial thrombus who requires urgent intervention.
