While you have accurately recognized the patient’s rheumatic mitral valve disease, rheumatic tricuspid valve disease is unlikely. The patient’s tricuspid regurgitation is likely due to tricuspid apparatus dysfunction from right ventricular dilatation due to severe pulmonary hypertension. In addition, rheumatic tricuspid valve disease is invariably associated with involvement of both the aortic and mitral valves.
