You are incorrect - the most likely diagnosis in our patient is aortic regurgitation.

Your choice: Aortic regurgitation and mitral stenosis

While you have accurately recognized the patient’s aortic regurgitation, the features of the apical diastolic murmur and heart sounds are not most consistent with mitral stenosis. Significant mitral stenosis would usually include a long diastolic murmur and opening snap and a loud S1. In addition, the right heart findings of mitral stenosis with significant pulmonary hypertension are absent.
