The combination of mitral stenosis and mitral regurgitation is almost always of a rheumatic etiology. In this patient, the murmur in early childhood is more typical of congenital rather than acquired heart disease.

The physical findings of a very wide pulse pressure and the findings on auscultation are incompatible with mitral stenosis and mitral regurgitation.
