Placement of a dual chamber pacemaker is inappropriate as the initial therapy for hypertrophic obstructive cardiomyopathy. In rare instances, it may play a secondary role in alleviating symptoms from left ventricular outflow tract obstruction through induction of dyssynchronous septal motion during ventricular systole.
