Global subepicardial injury


This electrocardiogram shows global subepicardial injury.

The characteristic features demonstrated here include: J point and concave ST segment elevations in most leads, such as leads I, II, aVL, aVF and V2 through V6. The T waves are upright in these same leads. J point and ST segment depression are seen in leads aVR and III only.

These features of generalized or diffuse ST segment elevation associated with upright T waves without reciprocal ST segment depression except in aVR, III and, rarely, in V1, are the classic findings in the early stage of acute pericarditis. The ST segment elevation occurs because the inflammation involves the entire adjacent epicardial layer of the myocardium.

This electrocardiogram shows some PR depression for example, in lead II also commonly associated with pericarditis.
